Long term care, healthcare ?

Who should be responsible for financing long-term care, the individual or the government? Why? It is widely accepted that the ability of the federal government to fund Social Security retirement, Medicare, and Medicaid is problematic for the long term. What do you think should be done to secure these programs for the future?
